TMR Medical Information
At Ten Mile River, we take safety as our highest priority. The mental, physical, and social health of our campers are of the utmost importance. Please use the resources below to answer any questions you may have.
BSA Annual Health and Medical Record
An annual medical examination and health record, completed within the past 12 months, is required and must be completed prior to your arrival. Print the TMR BSA Medical Form here.
A few notes specific to the TMR addendums to the medical form:
OTC Form (over the counter medication)
Ten Mile River employs a Reservation Nurse to oversee the health and safety of our campers. If a situation were to occur where a nurse determines OTC meds would be beneficial to their son or daughter, this form must be filled out. If parents would like their child to be eligible to receive OTC medication under the care of a nurse, this form must be signed by a doctor along with the medical form. Some examples of OTCs are benadryl, tylenol, or bacitracin.
Parents will elect on the form which OTC meds should or should not be allowed.
Meningococcal Meningitis form
Please note this form is only required for campers who spend two weeks at camp, (or anything over 7 nights).
Important Notes:
- NY State/county regulations require Ten Mile River to collect the original medical form signed by your physician and retain it. We will NOT be able to return your medical at the conclusion of camp. If you wish to use your medical for other purposes, be sure to make a copy of it BEFORE you come to Ten Mile River.
- According to New York State Department of Health regulations, all immunizations need to have Month, Day, and Year (Format: MM/DD/YYYY).
- Only the official BSA Medical Form (Parts A, B, C) will be accepted; no other types of physical will qualify. Please bring a hard copy of the completed form with you.
- Adults will be responsible to hold and administer your own medication. All youth must give any medication, either prescription or OTC, to the nurse during medical check-in. NY State requires a nurse to administer this medication. Adults in the unit may not hold and administer meds as they might during weekend camping.
Medication
All youth requiring medication, both prescription and OTC, must follow the following guidelines:
- Bring only the amount of medication needed for the duration of their stay
- Bring the medication IN THE ORIGINAL CONTAINER, which should state the name of the medication, frequency, and dosage.
- Make SURE the medication is listed on their BSA Medical form.
- During medical check-in, notify the nurse of your medication and hand it over to them.
Medication Times
Medication times are:
-BEFORE and AFTER breakfast
-AFTER lunch
-BEFORE and AFTER dinner
-Between 8 and 8:30pm
The nurse will assign your medication time during medical check-in based on your frequency requirements on the label.
SCOUTMASTERS: Use the TMR Medical Cover Form
.
We will be using this cover form to ensure all required parts of the medical are complete.
We highly suggest you print this off for each Scout and double check each section.
**Doing so will save valuable check-in time!
