Ten Mile River Medical Form
An annual medical examination and health record, completed within the past 12 months, is required and must be completed prior to your arrival.
Important Notes:
- NY State requires Ten Mile River to collect the original medical form signed by your physician and retain it. We will NOT be able to return your medical at the conclusion of NCS. If you wish to use your medical for other purposes, be sure to make a copy of it BEFORE you come to Ten Mile River.
- Sullivan County requires all camps in its vicinity to have all camp participants vaccinated for MMR (measles, mumps, rubella). Please be sure to include MMR VACCINE (two dates required) on Part B2 of the medical. Adults born prior to 1957 are exempt from this requirement.
- Only the official BSA Medical Form (Parts A, B, C) will be accepted; no other types of physical will qualify. Please bring a hard copy of the completed form with you.

Dietary Needs
The fabulous and tasty meals at Ten Mile River are prepared by year round professional chefs who work for Aramark, our food service vendor.
There expertise in meal preparation allows us the ability to accommodate any allergy or special diet (except for kosher).
Your meal is specially prepared and boxed separately with your name written right on the container, so you know what you’re getting every time.
